Historically, mental health services research has focused on high utilizers as the strategy to inform utilization management. While logical on its face, this approach makes an implicit assumption that at least some of these high utilizers are over-utilizing care. By using a comprehensive and robust assessment of an array of clinical and functional indicators, we can demonstrate that, at least for intensive community services for youth, that assumption may well be false. If you use a fully clinically informed assessment model, no youth in our large sample, used more services that would be expected based on their level of need. Rather the real problem is under-utilization.
