As an individual age, there is a high chance of getting false positive ANA results. ANA is measured with ELISA and an indirect immunofluorescence test (IIF). once ANA is positive, there is no point in repeating ANA titer because the change in ANA titer does not correlate with disease activity. With immunofluorescence, we can see a variety of staining patterns of ANA e.g. homogeneous, nuclear-speckled pattern, nucleolar pattern, and nuclear envelope pattern. • Depending upon the type of staining pattern, we can guess what type of ANA is present in a particular disease. • Homogeneous staining pattern suggests antibodies against DNA, DNA-histone, and histone. The speckled pattern suggests antibodies against U1RNP, Sm, La. The nucleolar pattern suggests antibodies against fibrillarin, RNA Pol I, III, PM-Scl, and RNA helicase. • anti-ds DNA is highly specific for SLE. • Ro, la antibody suggests Sjogren syndrome. Anti-scl70 antibody suggests diffuse systemic sclerosis. anti-centromere antibodies suggest limited systemic sclerosis.
