Anak ng isang OWWA member na active during the time of application. • 2. Kung ang active OWWA member ay single, wala pong asawa or married pero wala pong anak during the time of application, maari pong mag apply ang kanyang kapatid. • 3. Grade 12 graduating who is under 21 years old at may general weighted average of 80% pataas at kasama sa top 20 sa kanyang klase . • 4. Kailangan maipasa ang Department of Science and Technology – Science Education Institute (DOST-SEI) qualifying examination. • 5. Must be a Filipino citizen • 6. Must not be a recipient of any scholarship program, kailangan daw po ay wala pang nakukuhang kahit na anong scholarship kahit saan. • 7. Must be in good health and good moral character • For Second to Fifth Year College: • 1. Single • 2. Hindi hihigit sa 30 years old • 3. Anak ng isang OFW with an active membership at the time of application – kailanga po siguraduhin yong active member po kayo ng OWWA bago nyo iapply ng scholarship ang inyong mga anak para di po masayang ang inyong panahon • 4. kailangan po ang general average ng inyong anak ay di bababa sa 85% para di po sya madisqualified • 5. Must be a Filipino citizen • 6. Must not be a recipient of any scholarship award • 7. Must be in good health and with good moral character • Anu ano ang mga requirements for EDSP • 1. Accomplished application form (2 copies) • 2. 2x2 recent and identical photos (2 copies) • 3. Proof of relationship to an OWWA member kagaya ng • a. Kung anak nyo ang mag apply, kailangan po ng Birth Certificate (issued by PSA) ng anak nyo (original) • b. Kung kapatid nyo naman ang mag aapply, Birth Certificate (issued by PSA) nyo at ng inyong kapatid. Magbibigay ng birth certificate kasi po titignan nila kung tunay nga silang magkapatid. • c. Certificate of no marriage (CENOMAR of OFW) from PSA (proof of unmarried) katunayan po na single yong OFW na nagaapply para sa scholarship ng kanyang kapatid • 4. Para sa mga bagong first year college students, fill out Form 137 ito po yong card ng bata, at para sa mga nakaenrol na sa college, fill out Transcript of Records or Certificate of Grades • 5. Proof of OWWA Membership. Any of the following • a. Official receipt of OWWA Contribution - yung resibo po na katunayan na nagbayad tyo ng OWWA membership natin • b. OFW Verification Sheet issued by MPC – ito po ay makukuha sa POEA at pwedeng ang anak or asawa ang kumuha nito, magpunta lamang po sa POEA website para kumuha ng appointment. (https://ofwrecords.poea.gov.ph) • Paano mag apply ng OFW Verification sheet kung ang OFW ay nasa ibang bansa: If Requested by Family or Relatives of the OFW: • • Online Appointment Form - https://ofwrecords.poea.gov.ph/ • • Written consent of the OFW for the release of his/her Information Record • • Proof of relationship with the OFW – original or certified true copy of the following) • • Spouse – PSA Marriage Certificate • • Children – PSA Birth Certificate of the OFW’s child • • Parent – PSA Birth Certificate of the OFW • • Sibling – PSA Birth Certificates of both the sibling and of the OFW • • Valid Government Issued ID • 6. Health certificate (form 2A) • 7. Certificate of Good Moral Character issued by principal/guidance councilor (Form 2B) • 8. Certificate that applicant belongs to top 20% of the High School Graduating Class and that has a GWA average of 80% in his Grade 12 report card (Form 2C) • 9. Applicant’s certificate of not having taken post secondary or undergraduate/ college units and not a recipient of any scholarship grant / has not taken EDSP Qualifying Examination. (Form 2D) • 10. Sworn statement that he/she has no pending application for resident immigrant status in any country and does not have dual citizenship signed by parent or guardians (Form 2E) • 11. Copy of the scholarship agreement in its original form • How to apply for OWWA EDSP Program • 1. Kumpletuhin ang mga nasabing requirements. • 3. Ipasa ang lahat ng documents online or pupwede rin sa Regional Office na malapit sa inyong lugar. • Or pupwede rin ipadala sa mail ang application form, evaluation form and required documents sa courier services (LBC, J T, JRS etc). • 4.
