Dr. Ashraf Soliman describe the Essential Keratoconus Treatment Options Every Patient Needs to Know • #keratoconus • #Keratoconus_Treatment • If you, or someone you know constantly change your distance glasses or have high astigmatism and your glasses don’t give you the vision you hoped for ,then this video is for you. • I will walk you through the meaning of keratoconus its symptoms and how keratoconus is treated. • The cornea is a clear dome shaped lens in front of the eye and it has a regular smooth surface that bends the light rays entering the eye to focus them on the retina. It is part of a condensing system of lenses to allow this bending or refraction to happen and it has to be clear and have a very smooth surface to prevent these rays from scattering in the eye and defocusing the image. The cornea is made of tightly packed regularly arranged collagen bundles with cells lining the inner and outer surfaces . • In keratoconus the cornea becomes weak, loses its regular dome shape and becomes more steep and conical causing high astigmatism which results in scattering of light rays inside the eye. • The problem is that it is a progressive disease that usually starts at a young age and commonly related to constant eye rubbing and itching. • Many people with seasonal eye allergies may develop keratoconus and it needs to be addressed at young age and at an early stage of the condition to prevent complications • There are types or degrees of keratoconus according to the severity, visual acuity and how advanced the condition is. • The treatment modalities include • Cxl alone • Cxl and prk Athens or mod Athens • Cxl with icrs • DALK vs PKP • Let me give you an insight about each one • In Crosslinking corneal collagen fibers are strengthened by applying certain vitamin eyedrops called riboflavin and activating the vitamin by UV rays to create strong bridges and bonds between the collagen bundles making the cornea stronger and less liable to progress to a cone. • Cross linking can be done alone or with another procedure called topoguided PRK that aims to resurface the cornea and this procedure uses excimer laser • along with the strengthening Cxl. This is called the Athens protocol .this procedure allows clear vision with your prescription glasses and sometimes entirely ridding you of the glasses. • Cxl can also be done coupled with intracorneal ring segment implantation. These are rigid semicircular implants that are embedded into the corneal stroma or body through tunnels that are created using femtosecond laser technology. These flatten the cone and strengthen the cornea. • Some patients especially those that suffer from advanced keratoconus and corneal opacities require a corneal transplant.
