Meconium is passed during labour in some babies (as a result of fetal distress or postmaturity). When related to fetal distress, they may gasp and aspirate the meconium even before delivery. The aspirated contents have solid components that can obstruct airways (ball valve effect, overinflation and areas of atelectasis) and chemicals including the bile acids that can induce chemical pneumonitis, and also cause pulmonary vasoconstriction which increases the risk of PPHN in these babies. The inflammation also leads to a rise in CRP even in the absence of infection, which is something to keep in mind when it comes to duration of antibiotic treatment.
