PRP or platelet rich plasma has been an established treatment for the past four decades. It has been made famous by Kim Kardashian. Though she has said she regretted it because the treatment was painful (Does it take two neurons to synapse to understand that micro needling one's face without numbing cream hurts?) • . • . • Skin Science: This autologous treatment separates blood into 3 components, hemoglobin, platelet poor plasma PRP or platelet rich plasma. PRP has typical concentrations of 4.5 to 6.5X. Applications include- • . • Degenerative joint disease, musculoskeletal medicine • . • . • Dermatological uses: Hair loss, namely adjunctive mx of androgenic alopecia • . • . • Scar treatments: including acne, surgical, burns traumatic scars. • . • . • Wound healing: accelerated re- epithelialisation, reduces redness post inflammatory hyperpigmentation • . • . • Skin rejuvenation remodelling, namely the infamous vampire facial • . • . • Evidence in the context of scar remodelling: Numerous studies have shown contradictory results. Low numbers of subjects, largely uncontrolled. The few good studies again show conflicting findings. The problem with revision is the marked variation of scar types, treatment modalities, as well as treatment variables. Throw into the mix of variation of skin biology across ethnicities (the best studies conducted in the Middle East Asia), it is really hard to come up with a solid conclusion as to what really works • . • . • My 2 Cents: I did hundreds of PRP applications from late 2015 to 2017. All my insights are uncontrolled, because scar revision performed at a specialist level is bespoke hence uncontrolled. This means different treatments treatment parameters based upon the scar type skin type. My feeling- PRP gives marginal improvements (at best) in the context of recovery. PRP give zero-undetectable improvements in the context of end results, but PRP has a high patient satisfaction rate (the warm fuzzy bit of clinical practice) • . • . • Dr Davin Lim • @drdavinlim • Dermatologist • Brisbane.
