The thumb Ulnar Collateral Ligament (UCL) stress test is used to evaluate for a thumb UCL injury, often called gamekeepers or skier's thumb. The examiner grasps the thumb at the metacarpal and proximal phalanx maintaining the MCP joint in extension. The examiner then applies an ulnar stress by abducting the proximal phalanx relative to the metacarpal. The joint should be assessed in 30 degrees of flexion and full extension. Laxity at 30 degrees suggests proper UCL injury, in full extension, injury to both accessory and proper UCL. Comparison should be made to the unaffected thumb.
