During initiation or titration of allopurinol, I also top up with antiinflammatory agents like colchicine or NSAIDs. The reason for this is that if u prescribe allopurinol alone, it can precipitate acute gouty arthritis. Therefore, when we are initiating or titrating the allopurinol dose, we also top up prophylactically with anti-inflammatory agents.
