Go to https://www.swavlambancard.gov.in/ and open the home page of the website. • 2. Click on the link 'apply for disability certificate and UDID card' on the right side of the page. • 3. Enter name, DOB, father's name, mother's name, guardian's name of the applicant. Upload passport size photo within 30 KB, upload signature or thumb impression's photo within 3 to 30 kb. Enter communication address and permanent address, upload photo of address proof, select education qualification of the applicant. Finally click on 'next'. • 4. Enter disability details on this page. Select do you have disability certificate or not. Then select disability type, select disability area, select disability since or disability by birth, hospital treating disability, disability due to, select hospital etc. Then click on 'Next'. • 5. Select employment details in this page. Select if you are employed or not. then select occupation if employed, select unemployed since if unemployed, BPL/APL if any, select personal income etc. At last click on 'Next'. • 6. Select Identity proof, select aadhaar card, enter Aadhaar number, then select I agree to share aadhaar information with govt. department, enter captcha code, click on I have read and agreed the terms and conditions.
