Dr Nilesh Chordiya (a Gynecologic oncologist with over 10 years of experience in cancer surgery) answered one of our viewers' questions in this video. • A 64-year-old woman who underwent surgery for ovarian cancer a year ago has recently seen an increase in her CA-125 levels from 7 to 24 during a routine checkup. This rise in CA-125 levels is considered significant and often indicates the possibility of disease recurrence in either the abdominal or chest cavity. • To address this situation, it is recommended that she undergoes a PET scan, which will provide a detailed assessment of both the thoracic and abdominal cavities. If the recurrence is found to be localized within the abdominal cavity and is surgically removable, a comprehensive surgical procedure known as a peritonectomy may be considered. During this surgery, efforts will be made to remove any visible tumor deposits, potentially including parts of the bowel, in order to achieve an R0 resection, meaning that no residual tumor remains in the abdominal cavity. • In addition to surgery, hyperthermic intraperitoneal chemotherapy (HIPAC) may also be recommended as part of the treatment plan.
