Tyson Tripcony • In this ep of #DearDietitian, we go deep into the purpose of the Australian Dietary Guidelines, who follows them, when to use them, when to modify and when to use your clinical judgment. • We also answer one of our viewer's questions are the guidelines relevant to our clients? • A passionate topic for us this one, as you will see! • *** • Send us your questions - [email protected] or #DearDietitian. • *** • Continue the conversation: • Facebook: www.facebook.com/dietitianlife • Instagram: www.instagram.com/dietitian.life • Snapchat: www.snapchat.com/add/dietitianlife • Website: www.dietitianlife.com.au • Tyson's Linkedin: www.linkedin.com/in/tysontripcony • Peta's Linkedin: www.linkedin.com/in/peta-adams-1b060271 • Transcription • 0:20 – How relevant are the guidelines to clients? • The guidelines provide advice towards portions and serving sizes specifically for a healthy population • Most people in our population are considered to be healthy: meaning disease free, nothing wrong with them, good weight • Most people also don’t eat the same foods every day nor do they follow the guidelines • 1:50 – What about weight management? • If one leans towards the guidelines with their diet, they should maintain a relatively healthy bodyweight with minimal risk of developing disease and well nourished • It’s obviously different with those who for example have diabetes, then it can be modified • Only a 1/3 Australians eat enough fruit and veg and only 4% veg? • 3:30 – Who are the exceptions to the guidelines? • If you have intolerances, allergies, diabetes, heart disease, carrying excess weight, are underweight, exercise at higher levels than average, that’s a good opportunity for us dietitians to modify the guidelines to suit you as an individual • 3:50 – How to use the guidelines as a Dietitian • “It’s a guideline not a script” • As a dietitian, the guidelines are a good start, but they’re too general and people aren’t seeing you as a dietitian so you can regurgitate the guidelines • You have to use your clinical skills and adapt the guidelines so that the client can reach their goals e.g. increase exercise performance, gain muscle, which would be eating well above the guidelines compared to weight management who would be eating under • Not even dietitian completely know the guidelines by memory. • Nobody knows it’s actually a plate and not a pyramid • As dietitians we should always be modifying the guidelines as we don’t even generally see healthy clients • 6:05 – Using the guidelines as Nutritionist vs. Dietitian • Differences between these guys are that Dietitians give medical nutrition therapy tailored to the individual whereas Nutritionists are pretty much meant to follow the Australian Guide to Healthy eating…? But this certainly isn’t the case in reality • 8:00 – Bottom line: If a client asks about the guidelines what should you do? • If they don’t have any obvious healthy related condition then definitely use the guidelines as a starting point. • 9:10 – Why you shouldn’t mention the guidelines to a client • Otherwise don’t stick to the guidelines, especially with those with diabetes and weight management as the guidelines are definitely not targeted for • Guidelines terms can be confusing e.g. “discretionary” foods instead of “junk” or “processed” or “crap” foods • You might make recommendations that are exactly the same as the guidelines but even then, don’t feel the need to actually mention the guidelines to a client if you want them to come back to you as it’s boring as shit and you want them to have the impression that you actually have your own skills and knowledge by actually using them • 9:50 – An exception of when you should mention the guidelines • Only real exception is with kids and young children because you don’t want to change or manipulate or restrict their diets much. Plus it can be a good education tool for the parents on what they should be eating
