This is a case of a 22 year old male who presented with painful swelling of the calf. Frontal and lateral radiographs of the leg demonstrates a soft tissue mass in the posterior compartment of the leg with internal heterogeneous calcification. No definitite osseous lesion is identified. On the frontal radiograph, there is no convincing evidence that the knee joint is involved. In a a patient of this age, a periarticular calcified soft tissue mass is most consistent with synovial sarcoma. Synovial sarcoma is an aggressive malignancy that commonly occurs in the lower extemity.
