Posted daily. • https://plus.google.com/u/0/b/1139391... • • Management of portwine stains and red birthmarks • Apart from Salmon patches, Port Wine Stains are one of the most common red birthmarks, occurring in 2 out of 1000 births. • The majority of capillary vascular malformations are not associated with any other abnormalities, however there are some rare syndromes that are outside the scope of this video that maybe associated with vascular birthmarks. • One of the most common associations of vascular lesions is the emotional impact on the patient especially approaching the teenage years. With the event of safe, and effective lasers, all patient should have these lesions treated due to the fact that red birthmarks will thicken over time. • • Treatment options for PWS and vascular lesions • Lasers are arguably the best method for fading or removing PWS, with success rates approaching 90%. Good prognostic factors include early treatment, centro-facial lesions, birthmarks smaller less than 25cm squared lesions, flat red coloured lesions and PWS in fair skin patients. Extensive PWS on the limbs can be resistant to treatment. • I use 4 types of lasers to treat PWS- • Pulse Dye laser or V Beam- this is considered the Gold Standard of PWS treatment. It has a wavelength of 595nm and is highly attracted to the haemoglobin in PWS. The heat energy from the laser is preferentially taken up by the port wine stain, and less energy is taken up by the normal skin, thus allowing for selective treatment of PWS. This is a process is called selective photothermolysis. • 1064 nm Long Pulse Nd Yag- this wavelength is longer, and hence travels deeper into the skin, and can be useful for treating ‘blebs’ of older PWS, and thicker lesions. I sometimes combine the V Beam with the 1064 in the one sitting. • 755 Alex laser- can be employed for deeper lesions, but also in darker patients who have PWS, as melanin can be an issue with shorter wavelengths. • CO2 laser- occasionally long standing lesions can be so thick, that vascular lasers are not effective. This laser is used to flatten the PWS, enabling vascular lasers to target haemoglobin. • Surgery, sclerotherapy and diathermy can be employed to treat lesions that may exhibit blebs, or associated AV malformations. I work in with a radiologist and a vascular surgeon to mange these lesions. • • Medical options and make-up • Cosmetic make up including Dermablend can provide excellent cover up for patients who have even the darkest discolouration from PWS. Colour matching is the key. We suspect that the ideal treatment of resistant PWS maybe a combination of lasers, topical creams that act as vasoconstricting agents, and skin matched make-up. • • Thanks for watching this video on treating birthmarks, one new video every Saturday.
