In this video I would like to explain some aspects and clarify when lumbar MRI is necessary. • The MRI of the lumbar spine is used mainly for the study of lumbar disc hernias and when it is connected to sciatica lumbar or low back pain. • Sciatica lumbar is pain that affects the lumbar tract of the patient's spine and radiates, also affecting the legs. • The importance derives from the fact that numerous pathologies are related to the crushing of the nerve roots that provide sensitivity and motor skills to all areas of the lower limbs. • To understand it better, when your legs and toes fall asleep, when you feel a tingling sensation in your foot, and when you have sensitivity problems. These symptoms are usually due to compression or inflammation of the nerve root. • In fact, we call sciatica, or rather sciatica lumbar, the intense pain that comes from this compression. Only with a certain diagnosis will it be possible to carry out the appropriate therapy. • Responsible for this compression is the vertebral disc that acts as a cushion that is interposed between two vertebrae and that serves to cushion movements and make the spine flexible and mobile. • When this cushion is crushed by the reduction of the space between the vertebrae, and in the posterior area, it can compress and inflame the nerve roots. • Low back pain, on the other hand, is more frequently called back pain, it is a common disorder that involves the muscles and bones of the lumbar spine. It affects approximately 40% of people at some point in their lives. Low back pain that lasts for at least a day and limits activity is a very common disorder. It is estimated that worldwide between 40 and 80% of people experience at least one episode of low back pain at some point in their own life. This condition usually begins between the ages of 20 and 40 and is most common between the ages of 40 and 80. The total number of people affected by low back pain is estimated to increase as the population ages. • The main international guidelines for the treatment of back pain distinguish between pain in the acute phase, that is, present for less than 6 weeks, and pain in the chronic phase, present for at least 12 weeks. • For the clinical setting, in both phases, there are no studies that demonstrate how subjecting a person with back pain to radiological investigations, such as X-rays or MRI, actually helps to improve their low back pain. • The international scientific community strongly advises against the use of these radiological investigations in all people with back pain. • That said, the request for a lumbar MRI is made by the doctor who knows the patient's medical history and is in possession of all the information to be able to request, if deemed necessary, a lumbar MRI. • It is not my intention to promote MRI tests because, as I explained above, it is a strictly medical decision.
