14 years boy shows improvement in just 3 months after treatment for hereditary spastic paraplegia (HSP) by dr alok sharma, mumbai, india. • After Treatment • 1. Oromotor skills improved – tongue movements have improved, improved mouth closure is seen. • 2. Oromotor speech improved in terms of clarity. • 3. His sitting static and dynamic balance has improved in terms that he can reach for objects from the floor, he can cross his leg and sit on chair. • 4. His standing static and dynamic balance has improved. He can stand without support for 10 seconds and with support for 1/2 an hour. • 5. His walking balance has improved. Walking indoors he needs only supervision and walks with help of walker with push knee splints and boots. His walking speed has improved. He can walk 13 feet distance for approximately 10 single rounds. • 6. His gross limb movements have improved. Overhead activity and distal hand movements have improved. • 7. Fine motor – handwriting and drawing skills have improved. He can eat his food by himself with his own hand. He can tear chappati also. • 8. Involuntary movement like knee and ankle clonus have also reduced and adductor scissoring also has reduced due to decrease in muscle tone from grade 3 to grade 2 in the lower limbs. • 9. His knee range of motion has also improved from 10 degrees extension to complete flexion. Earlier it was 30 degrees extension to complete flexion. His TAs are also completely stretchable. • 10. His posture has also improved. Earlier he was doing sacral sitting, now he sits properly in a much corrected way and can correct his posture when asked. • 11. His overall stamina and endurance has improved. • 12. His attention and concentration has improved. • 13. His new learning and understanding has improved. • 14. Transfers – now he his independent for same level bed to chair transfers and needs minimal assistance from sit to stand and vice-a-versa. • 15. His dissociation reactions have improved. He can now do trunk-pelvis and femoral pelvis both. • 16. Score has improved on Berg Balance scale from 13/56 to 21/56. • 17. Voluntary control of hip and knee has improved. Voluntary control of ankle and toes was poor, now some flicker movements are seen. • 18.
