Braden Scale is a standardised evidence based assessment tool used to assess patients at risk of developing pressure related injuries. It was developed by Barbara Braden and Nancy Bergstorm. It is a silent station in OSCE, you have 8 minutes(subject to review) to complete this station. • 6 risk factor areas to consider which include sensory,moisture,activity,mobility,Nutrition,Friction and shear. Each risk factor is rated on a scale of 1-4 excluding friction and shear with a rating of 1-3.
