Hi!!! Seating at mealtimes doesn’t always have to be upright, structured, and 90-90-90! • 🤯 Wait, what?! • Here’s the thing… the idea behind promoting 90-90-90 positioning is good - we want stability of the head, neck, and trunk while eating to decrease risk of choking or other adverse responses (e.g. aspiration) • HOWEVER, there isn’t any research that tells us it HAS to be THAT position. • 🚫 Plus, it isn’t always the most feasible or supportive option for a child. • ➡️ Have you ever had a child on your caseload with spastic quadriplegic cerebral palsy? What about a little one who just can’t stop wiggling? Or a child who doesn’t sit up on their own at 10 months even though you’re suppose to be starting solids?! • 👏 We should not delay starting solids or restrict oral feeds just because 90-90-90 cannot be achieved. We need to pivot and modify to make things work for THAT child. • Here are some other ways you may consider supporting positioning during feeds ⬇️ • -Use pool noodles or rolled up towels for lateral support • -Allow some flexibility with seating (as long as they are safe), such as sitting on knees, dangling feet/legs so they can bounce on a band strapped to the chair, or use a wobble cushion. • -Slightly recline seating for better head/neck support and control • -Consider adaptive seating (e.g. Rifton, special tomato) for additional support • - of course, collaborate with OT and PT to get in the most supportive position achievable! • ✨ If you relate or have more ideas, share them ⬇️ • #90-90-90 #NinetyNinetyNinety #Positioning #Feeding #PediatricFeedingTherapy #PediatricFeedingSLP #SLP #SpeechLanguagePathology • Check out my website (https://pediatricfeedingslp.com) and Instagram (  / c_wsdpzxwsd  ) for more info!
