Cervical hydrops due to stenosis in a young adult female presents on ultrasound as a distended cervical canal filled with anechoic or hypoechoic fluid, with thickened cervical walls and narrowed external or internal os. Color Doppler typically shows no internal vascularity within the fluid, though increased vascularity may be seen in surrounding tissues if inflammation or neoplastic processes are present. After six months of conservative management, follow-up ultrasound reveals a reduction in fluid collection and partial restoration of cervical patency. Common causes include congenital abnormalities, infections, or post-surgical scarring. Differential diagnoses include cervical mucoceles, Nabothian cysts, endometrial polyps, and, less commonly, hematometra or cervical cancer.
