Bad Endings. I don't know about anyone else, but they terrify me. Just when the hero(es) thought everything was finally right, just when it looked like peace had finally returned to the world/city/etc, something had to happen that, at best, creates a bitter sweet victory and, at worst, spoils everything and the enemies win after all your struggles. Sure, Game Overs CAN be scary too, but more often than not, all you'll get when you run out of lives is a simple Game Over message, possibly with the option to try again.
