Viral pharyngitis is characterized by fever, sore throat, cough, pharyngeal erythema, and non-tender adenopathy. Other suggestive findings include hoarseness, conjunctivitis, and diarrhea. Frank exudates CAN occur with viral etiologies (e.g., adenovirus, Epstein-Barr virus). If a viral etiology is suspected, then neither a throat culture nor rapid antigen detection test is indicated. It must be realized that some patients with viral pharyngitis will indeed have streptococcal bacteria in their throat. The presence of the bacteria does not indicate infection, but colonization. Since neither throat culture nor rapid antigen detection tests can distinguish between infection and colonization, these tests should only be ordered when sufficient suspicion exists, and should NOT be used as a test of cure. Treatment is supportive.
