Tooth Extraction for Orthodontic Treatment causes TMJ Jaw Pain

People who have had orthodontic treatment can develop Temporo-Mandibular Disorder (TMD) symptoms later as a result, and can end up suffering in pain without realizing that the orthodontic treatment contributed to their TMD. • In some cases of orthodontics, including braces and Invisalign, bicuspids are extracted when there is a lot of crowding, in order to make space to align the remaining teeth. But this can lead to a whole number of issues later on in life and many patients come into my office suffering with TMD that is a result of having bicuspids extracted. Why would tooth extraction cause TMD? • In order to answer that, we need to define what a well-developed jaw is first. In a well-developed maxilla, teeth erupt into position, and form a U-shaped arch. If the bicuspids are removed in a crowded tooth situation, now there is room to align the teeth, so many orthodontists choose to bring front teeth into position using that space. Whether it’s braces, Invisalign, clear aligners, or any other form of tooth movement, if front teeth are reclined or moved back to align and close the space, this can be a problem. • When anterior teeth are reclined and tipped back, that locks the mandible and causes it to be moved back along with the upper teeth. As the mandible moves back, it distalizes the condyle. The retruded mandible and condylar head can then result in damaged tissue and ligaments that hold the articular disc in position, and the articular disc can anteriorly displace. This is known in dentistry as disc displacement with reduction. This is when the person will start to feel or hear a click or a pop when they open and close. Overtime, this can lead to a whole number of TMD symptoms including jaw pain, muscle tension, locked jaw, difficulty opening or chewing, ear pain, ear ringing, and ear fullness. As the damage continues, the click may disappear, as the disc no longer pops back onto the head of the condyle, and this is known as disc displacement without reduction. • Patients suffering with TMD symptoms as a result of orthodontic treatment that retruded their mandible may need reversing the treatment in order to allow the mandible to come back into its correct position.
