Provides analgesia for lateral chest wall. Useful block for analgesia following Mastectomy and other breast surgeries. Has been recently used for analgesia for fractured ribs as an alternative to paravertebral or thoracic epidural. • Initially described by Gaur as a sub-serratus injection in an editorial. • http://www.gosipper.com/Journal/Artic... • Gaur approach involves: • SAP block is done with the patient in the lateral position, injection in the post axillary line (where SA muscle is well formed) with needle directed posteriorly and cephalad. • (a) with needle on top of rib beneath Serratus Anterior and • (b) with needle between external intercostal muscle and Serratus Anterior • Also, Gaur used normal saline to open the space and then inject LA. • Later described by Blanco et al as an above the serratus injection in their volunteer study (They described both injection above and below the serratus, but the analgesia was prolonged in the above the serratus injection). • http://onlinelibrary.wiley.com/doi/10... • Fajardo et al have described this block as the Serratus Interfascial plane block (Anterior and Lateral approaches). • http://www.respond2articles.com/ANA/f...
