There are two major criteria for infective endocarditis. • The first is a positive blood culture for endocarditis. This criteria is established in one of three ways; • 1. If typical microorganisms consistent with IE are found on two separate blood cultures • 2. If blood cultures are persistently positive • 3. If a single blood culture is positive for Coxiella burnetii or a phase I IgG antibody titer is greater than 1:800 • The second major criteria is evidence of endocardial involvement. This criteria is established in one of two ways; • 1. If an echocardiogram detects a vegetation, an abscess, or a new partial dehiscence of a prosthetic valve. • 2. If there is new valvular regurgitation. • There are five minor criteria for endocarditis. • 1. A predisposing factor such as iv drug use or a predisposing heart condition • 2. Temperature greater than 38oC • 3. Vascular phenomena such as a major arterial emboli, mycotic aneurysm and janeway lesions. • 4. Immunologic phenomena such as osler nodes, roth spots, and positive rheumatoid factor. • 5. Microbiologic evidence such as positive blood culture not meeting a major criterion or serologic evidence of an active infection with an organism known to cause infective endocarditis. • A definite diagnosis of IE can be made with one of three combinations of the criteria mentioned: • 1. Two major criteria • 2. 1 major and 1 minor criteria • 3.
