How about Low Molecular Weight Heparin? Aka Lovenox (Enoxaparin) • As the name implies, Lovenox is a smaller version of heparin on the molecular level. It has a similar mechanism of action, enhancing antithrombin III’s activity, but has more of an effect on Factor Xa than on thrombin. • Similar to heparin, it is given subcutaneously, however there is never an indication for IV administration of lovenox like there is for heparin. • In terms of dosing, lovenox for DVT (deep vein thrombosis) prophylaxis is typically dosed once daily, whereas heparin is dose twice or three times daily. Lovenox is also has more bioavailability and a predictive effect compared to heparin, therefore requiring less monitoring and titration of dosing with it’s use. This is particularly useful for achieving therapeutic levels of anticoagulation for patients with an active clot or who are at high risk for clot formation (ie. Atrial fibrillation, mechanical heart valve, etc.). • Lovenox is the preferred agent for pregnant patients who require anticoagulation. • The one place you would want to avoid lovenox is in patients with renal failure because they will accumulate levels of the medication since it is renally cleared. Another situation where you may prefer heparin over lovenox is if you have a patient that you anticipate will need surgery or a procedure at any given time. With lovenox, you typically have to wait a longer period of time from the last dose to when you can assume the anticoagulation is no longer a concern.
