We want baby to be vertex, and most babies are! Why are babies vertex? Because that is the smallest part of the head which can push on the cervix to dilate it and thus the greatest chance of success for vaginal delivery. However, some babies are positioned in Transverse, Breech, or Oblique. What do we do? Most providers take you to C-Section at 39 weeks to make sure you don’t rupture your membranes and end up with an umbilical cord prolapse. But you don't have to go straight to C-Section. • Some women with a baby that is breech position don't want a C-Section. You do have another option. It is called an External Cephalic Version (ECV) that is to rotate your baby from breech to vertex. Obstetricians can do ECV if they feel comfortable with them. ECV have a 50% success rate. However, they can only be successfully done on babies that are in the Frank Breech presentation (butt down), transverse position, or oblique position. Talk to your doctor or look in your local area. • How it it done? You will go to the hospital operating room and be administered spinal anesthesia. If there is any problems with baby we proceed with C-Section. We use ultrasound guidance, apply gel to your belly, and try to rotate baby. The baby will stay in that position (over 90% of the time). • Some doctors do it at 39 weeks and induce you at the hospital for delivery. Some doctors do it at 36 weeks because baby smaller and there is adequate amniotic fluid for more movement. This has a higher success rate.
