Carbachol is a direct acting cholinergic agonist and it can act on both muscarinic and nicotinic receptors. It is ester of carbamic acid. It dont have methyl group unlike Bathanechol. It act on Cardiovascular and GI tract stimulating and then depress these system. It is rarely used except ophthalmologically, where it is use to treat glaucoma as it causes contraction of ciliary muscles and decrease in intraocular pressure. it is use as miotic agent. It cause little or no adverse effects as it cannot penetrate system when used ophthalmically. Carbachol is potent, non-selective and has long duration of action.
