BOU – Benign Oral Ulceration • Caused by: trauma, stress, food products (chocolates, nuts), medications (NSAIDs, Beta Blockers), Vitamin deficiencies. • Immunological process. • Pain exceeds the size of the lesion. • Minor, Major, and Herpetiform aphthae. • Minor Aphthae: • 80% are less than 1 cm. • Generally located on labial or buccal mucosa, the soft palate, and the floor of the mouth. • Can be singular or multiple, small (less than 1 cm in diameter), and shallow. • Major Aphthae: • Larger and involve deeper ulceration. • More likely to scar with healing. • Herpetiform Aphthae: • More numerous and vesicular in morphology. • Note: Patients with benign aphthous ulcers should have no other findings such as fever, adenopathy, gastrointestinal symptoms, or other skin or mucous-membrane symptoms. • Treatment includes: • Avoidance of triggers. • Local pain relief. • Chlorhexidine gargles. • Vitamin supplements. • Oral steroids (rarely). • ORAL PEMPHIGUS VULGARIS • Presents with oral lesions in 50-70% of patients, and almost all patients have mucosal lesions at some point. • Mucosal lesions may be the sole sign for an average of 5 months before skin lesions develop, or they may be the sole manifestation of the disease. • Diagnosis should be considered in any patient with persistent oral erosive lesions. • Intact bullae are rare in the mouth. Patients more commonly have ill-defined, irregularly shaped, gingival, buccal, or palatine erosions, which are painful and slow to heal. • The mucous membranes most often affected are those of the oral cavity, sometimes the only area involved. • Treatment includes: • Topical application. • Administering analgesics. • Maintaining strict oral hygiene using diluted antiseptic mouth wash. • Periodontal treatment. • Following a soft diet without irritants. • Checking prosthetic restorations. • Applying anti-candida therapy in patients on long-term corticosteroid treatments. • Research advances include treatments such as: • Pulse therapy (intravenous infusion of very high doses of Immunosuppressants for a short time period). • High doses of intravenous immunoglobulin. • Plasmapheresis. • STEVEN JOHNSON SYNDROME • An immune-complex–mediated hypersensitivity complex typically involving the skin and mucous membranes. • Classification includes: • Stevens-Johnson syndrome: A minor form of toxic epidermal necrolysis, with less than 10% body surface area (BSA) detachment. • Overlapping Stevens-Johnson syndrome/toxic epidermal necrolysis (TEN): Detachment of 10-30% BSA. • Toxic epidermal necrolysis (TEN): Detachment of more than 30% BSA. • TRAUMATIC ULCER • Usually caused by injury and is self-limiting. • Chronic trauma, as caused by Paan, Tobacco, Gutkha, sharp, or infected tooth can cause a more dangerous type of ulcer. • The ulcer is indurated with raised margins, sometimes causing cervical lymph node enlargement, leading to severe, unremitting pain.
