C5 Herniated Disc Evaluation and Management

Here is a cervical radiculopathy evaluation that I did on a lady who developed cervical radiculopathy with left upper trapezius pain and soon after, significant weakness of the deltoids, biceps and supinator. She has decreased C5 and C6 deep tendon reflexes and decreased sensation to the C5 dermatome.
