Up to 46:26, the leadup is identical to the regular neutral ending but if you did everything correctly to unlock the secret ending then Nuwa should appear here and give you a new option, to create a world for humans. In order to be truly sure, you can just complete every possible quest like I did in my playthrough. Unlike SMT IV, you don't get penalized for taking on certain law/chaos quests because once again you can choose your final alignment before the Empyrean.
